Navigation Services
The Navigation Services Program provides the uninsured and underinsured population of Calhoun County the resources necessary to stay healthy. These resources include: enrollment into another program at CHC ( Prescription Drug Access Program, Lions Club Eyeglass Program), help with obtaining durable medical equipment through the Health Assistance Fund (HAF), help with medical transportation through the Health Assistance Fund (HAF), community referrals, patient advocacy services (help with filling out applications, paperwork, understanding current options, making doctor appointments, finding a primary care provider (PCP), prescription medication co-pays, doctor visit co-pays), help with basic medical coverage through Affordable Care Act Marketplace and providing eligibility determination and enrollment, help with state insurance enrollment services such as Medicaid or Healthy Michigan, and even lending an ear to those who need to be heard. The Navigators are the “connectors” of the community, in helping patients and clients gain access to medical care and health-related goods and services that they can’t access on their own.
Navigators provide a great deal of medical education to patients and clients who seek help. Each Navigator is trained and certified to provide help with obtaining medical coverage through either Marketplace or Healthy Michigan, and can help patients to truly understand what type of plan they are enrolled in. Many patients who have purchased what they though was an affordable insurance coverage plan through the Marketplace, are finding that they are unable to pay their co-pays, meet their deductibles, or even afford to use their coverage. Navigators can help these individuals to learn about the options that are available to them, as well as how each option will affect their income and their family. Navigators take the time to sit with each patient, listen to what struggles they are going through, and do whatever it takes to help fulfill that individual’s need. The staff is proud to work each day under the philosophy of “No Missed Opportunities”, which means that the staff will stop at nothing to ensure that each patient’s need is not only fulfilled for that day, but that each patient will receive a respectful service that will allow them to be able to help themselves in the future. The goal is not only to fix what the issue is TODAY, but help the patient to fix the issue FOR GOOD.
Navigation Services not only helps those who come into our office for care, but also reaches out to the community in order to provide care to those who cannot make it to our office for a variety of reasons. Navigators, as well as our staffed Registered Nurse in most cases, visit local churches and senior apartment complexes to deliver care where needed. Navigators also often provide assistance for patients in other doctor offices through care management referrals.
